Panniculectomy surgery in London aims to remove the ‘pannus’ or ‘apron’ from the lower abdominal area.

A pannus is a portion of excess fat and skin that hangs from the lower abdominal area over the groin, similar to an apron. A panniculectomy is done through an incision that extends from hip bone to hip bone and is a different procedure to a tummy tuck (or ‘abdominoplasty). The aims of panniculectomy are to improve the contour of lower abdominal area, lead to improved personal hygiene and facilitate physical activity.

A panniculectomy operation can be done as an outpatient, meaning that you can go home the same day following the surgery. If the surgery is extensive, or if there is any need for extended observation following the general anaesthetic, you may need to stay in a hospital overnight.

Mr Ioannis Goutos has a special interest in body contouring, as well as scar management and burn care. In addition to being an expert and skilled surgeon, Mr Goutos has a keen interest in the psychodynamic aspects of plastic surgery and cosmetic procedures. Treatments with Mr Goutos include full consultations, discussions of treatment options, including risks and benefits of each, as well as the surgery itself and the post-operative recovery.

What is panniculectomy surgery?

A ‘pannus’ or ‘apron belly’ can arise from excessive weight gain/loss from around the abdomen. The abdominal wall is made up of layers of fat both overlying and underlying the ‘six-pack’ (rectus abdominus) muscles. If there is excess fat in this area, this, and the overlying skin, hangs over lower groin/pubic area, it looks like an apron, hence the term ‘apron stomach’. If diet and exercise do not improve the appearance, then a body contouring consultation may be appropriate and a panniculectomy procedure can be discussed and considered.

Panniculectomy is an operation done under general anaesthetic, meaning that you will be asleep during the procedure. Once safely asleep, you are monitored throughout the operation by an anaesthetist. The procedure is done by making a horizontal incision from hip bone to hip bone and followed by methodical removal of the excess skin and fat. The full length of the incision will depend on the amount of tissue that needs to be removed. Once the excess tissue is removed, the remaining skin is carefully joined together using stitches.

Following the surgery, most people can return to light work or office-based work after a few days, but will need to wait for several weeks before re-introducing any heavy lifting or strenuous activity or exercise. Recovery back to normal activity can take a few months. You will need regular checks and follow up during this time to ensure that the recovery process is going smoothly and that the surgical scar is settling well.

Frequently Asked Questions about panniculectomy

We know having medical, surgical or cosmetic treatment can seem daunting and it is natural to have questions. We are here to ensure your comfort and safety at all times. If you are unable to find the answer to your questions below, then please do not hesitate to contact us so we can help.

Individuals who should consider getting a panniculectomy typically include those who have experienced significant weight loss, resulting in excess skin and fat in the abdominal area that cannot be addressed through diet or exercise alone. This procedure is particularly beneficial for patients suffering from physical discomfort, such as chafing or infections caused by the hanging skin, as well as those whose daily activities are hindered by the presence of a pannus. Additionally, individuals with specific medical conditions related to obesity may find that a panniculectomy not only improves their quality of life but also enhances their overall health by facilitating better mobility and hygiene. It is essential for potential candidates to consult with a qualified plastic surgeon to assess their individual circumstances and determine if they are suitable for this surgical intervention.

The main risks of this surgery are similar to those from any surgical procedure, including bleeding, swelling, scarring and possible infection at the surgical site. There is also a small risk of seroma (i.e pockets of tissue fluid under the skin), as well as forming a blood clot. Many of these risks will be dependent on the health and medical background of the individual having the procedure; they can be reduced by ensuring appropriate pre-operative assessment and planning and post-operative care.

The recovery time for a panniculectomy typically ranges from four to six weeks, although individual experiences may vary based on factors such as the patient’s overall health, the extent of the surgery, and adherence to post-operative care instructions. During the initial recovery phase, patients are usually advised to take it easy, avoid strenuous activities, and gradually increase their mobility as they heal. Most individuals can expect to return to light daily activities within two weeks, while more vigorous exercise and heavy lifting should be avoided for at least six weeks. It’s essential for patients to follow their surgeon’s specific recommendations and attend follow-up appointments to ensure a smooth recovery process.

Although the primary aims of a panniculectomy do not include weight loss, you can expect a degree of weight loss depending on the amount of tissue removed during surgery. Most people will notice a significant improvement in their ability to do physical activity and so will continue to lose weight in this way.

It is important to allow adequate rest and recovery following a surgery like this to facilitate good healing of the surgical wound. This includes avoiding certain activities including lifting heavy objects and straining. Taking painkillers and ensuring good skin hygiene around the wound can also help improved recovery.

Whilst it takes up to a few months for full recovery, the complete end results may take up to a year to fully appreciate. During this time, the scar  becomes flatter and loses its florid red colour; regular follow up is important to ensure that the best results are achieved and to prevent recurrence of the ‘apron stomach’.
